- 樣本：共納入 37 名 PML 患者，其中 23 名有血液惡性腫瘤；每人輸注中位數 2 劑（range 1–12），追蹤 12 個月。
- 關鍵數字：21人（56.8%）達 overall response，其中16人（43.2%）為 complete response；反應中位時間23天，且未見反應喪失。1年 OS 為61.1%（95% CI 41.9–77.4）；首劑即有反應者與未反應者的1年存活率為93.3%與0%（p<0.001），但此比較屬治療後反應分層，不能視為預先可用的因果效應。
- Limitation：單中心、樣本小且無對照組，無法排除病人選擇、免疫抑制調整及支持性治療造成的 confounding；以病毒清除合併神經學穩定或改善定義反應，也可能受評估方式與自然病程影響。
- Bottom line：對缺乏核准治療且死亡率極高的 PML，第三方 BK-VST 顯示快速且持久的臨床訊號，值得進一步以多中心研究確認並建立早期反應判準。

- Bottom line：實體器官移植者的癌症死亡約為一般人口兩倍，應依移植器官、癌別與地區調整監測，尤其重視 non-Hodgkin lymphoma，而非全面加強所有癌症篩檢。

- Limitation：回溯性、樣本小且僅來自兩家醫院，治療選擇未隨機化；病況較嚴重或治療反應不佳者更可能接受合併治療，存在明顯 indication bias 與殘餘 confounding，故不能據此主張單一治療優於合併治療。
- Bottom line：isavuconazole 在血液惡性腫瘤合併 IMD 的實務使用中具可接受療效與良好耐受性，但 mucormycosis 成效較差，且治療策略比較須避免因未校正的病情差異而過度解讀。
