- 樣本:分析對象橫跨血液惡性腫瘤與實體腫瘤 ADC,標靶包括 CD30、CD79b、CD22、CD33、CD19、BCMA、HER2、Trop-2、Nectin-4、Tissue Factor、FRα、c-Met;關鍵 phase 2/3 試驗每組約 57–1,486 人,但追蹤時間與感染定義不一致。背景證據包括 169 個 ADC 臨床試驗、超過 22,000 名病人的 meta-analysis,以及 FAERS 24,618 件 ADC 相關通報案例。
- 關鍵論點:感染風險高度異質,calicheamicin 類最高:GO 在 ALFA-0701 induction 的 grade ≥3 infection 為 47% vs 39%,InO 在 R/R ALL 的 grade ≥3 infection 反而低於化療 28% vs 54%;FAERS sepsis 訊號以 GO 最強 ROR 28.52(95% CI 26.80–30.35),InO ROR 10.53(95% CI 9.22–12.01),整體 neutropenic sepsis ROR 16.34,sepsis 相關死亡約 30%、multiple organ dysfunction syndrome 達 40.1%。Vedotin 類除 neutropenia 外還有 PML、PJP、CMV、herpesvirus 與 fungal infections;實體腫瘤 ADC 整體較低但有器官特異性,例如 EV grade 3–4 UTI 5–12%、T-DXd respiratory infection 0.8%、SG FN 4–8% 與 neutropenic colitis 1.4–2%,顯示效應量在血液腫瘤與特定 payload/linker 組合中很大,但跨藥物不可直接外推。
- Limitation:主要限制是資料來自 prescribing information 與 pivotal trial 安全性摘要,感染事件並非所有研究都系統性收集,且 monotherapy、combination therapy、疾病本身免疫缺陷與既往治療暴露會混雜 ADC 歸因。FAERS 可偵測不成比例通報但有 reporting bias、不能證明因果;跨試驗比較受族群、AE 定義、樣本大小與 2020–2023 年 COVID-19 pandemic 影響,特別會干擾 pneumonia/COVID-19 pneumonia 的解讀。
- Bottom line:臨床上不要把 ADC 視為單一感染風險類別,而應依標靶、payload、linker、疾病背景與併用治療分層:GO/急性白血病按高風險 neutropenia 管理,Pola 明確考慮 PJP 與 herpesvirus prophylaxis,BV 注意 PML、PJP、CMV 與 HSV/VZV,SG 在前幾個療程評估 G-CSF 與 UGT1A1 風險,因約 60% ADC-associated sepsis 發生於治療第 1 個月內。

- Limitation:最大限制是期中分析且樣本僅 14 人,沒有隨機分派或同期對照,與歷史對照比較容易受 baseline risk、移植照護改變、監測頻率與 preemptive therapy 門檻影響。研究主要呈現 feasibility 與 safety signal,對 CMV reactivation 降低、cGVHD 風險與死亡等臨床終點都明顯 underpowered。
- Bottom line:這篇值得注意的是,快速自動化製備 prophylactic CMV-specific CTLs 在 allo-HSCT 後早期輸注看起來可行且短期安全可接受,但目前只能視為 hypothesis-generating,尚不足以取代 letermovir 或改變常規 CMV 預防策略。
